Log in to your JetBrains Account with the email you used for the Student Pack.

Open the store and pick an IDE or the All Products Pack on a personal subscription.

Check the cart: the 40% graduation discount should already be applied before you pay.

It applies to personal subscriptions only, not company licences.
It ends two years after your Student Pack expires, and then you pay the normal price (US$299 a year for the All Products Pack in year one).
The store card for this offer shows US$179.40 against US$299.00, so confirm the cart total before paying.

Do I need to apply for the graduation discount?

No. JetBrains applies it automatically when you log in with the account that held the Student Pack.

Does the 40% stack with the renewal discount?

The official pages don't say whether the graduation discount stacks with the 20% and 40% continuity discounts.
